Coming Home Preparations
Discharge & Home Support - Going home with a preterm baby is both joyful and delicate. PIPNU ensures you leave the hospital confident and prepared.
Before You Leave Hospital:
- Discharge planning and care training (medications, feeding, breathing, danger signs)
- Equipment orientation (thermometer, KMC wrap, emergency contacts)
- Connection to your Village Kangaroo Ambassador, a trained parent mentor
Your First Days at Home:
- 48-hour home visit by your Ambassador
- 24/7 Parent Support Line
- Guidance on feeding, nutrition, home safety, and infection prevention
Ongoing Follow-Up:
- Weekly visits (weeks 1–4), bi-weekly (months 2–3), monthly (months 4–12), quarterly check-ins (year 2+)
- Monitor growth, milestones, nutrition, danger signs, and parent well-being
- Secure tracking of your baby's health records
Specialized Services:
- Monthly preterm wellness clinics
- Developmental screenings at key ages (4–36 months)
- Vision, hearing, and vaccination monitoring
